Patient Admissions at Epworth Freemasons

Admissions to the Hospital are processed through our Bookings Office. Your doctor will by now have made arrangements for your admission to Epworth Freemasons and you will already have been provided with a Patient Registration and Consent Form. This form should be completed by you and forwarded to the address on the form. It is important that you accurately complete the health insurance section of your Registration Form. By doing so it may save you a great deal of time and unnecessary worry.

Epworth Freemasons will endeavour to contact all incoming patients prior to admission. On the day of your admission, please report to Reception at the designated time. The information you provided on your Patient Registration Form will be discussed with you on admission.

It is important to take all your usual medications (regardless of fasting) with a small amount of water at the usual times, unless advised otherwise by your doctor. If you have diabetes you should contact your doctor for specific instructions regarding your diabetic medication (i.e. Insulin or tablets). Bring your medication and testing equipment with you.

Patient rights and responsibilities

It is important that you are aware of your rights and responsibilities as a patient. A copy of your Rights and Responsibilities will be sent to you prior to admission, or given to you on admission.

Paying your account

Your doctor will provide you with an estimate of the anticipated costs of your hospitalisation. On admission, our staff will provide further details and ask that you sign a Financial Consent Form.

Epworth Freemasons will submit a claim to your health insurance fund on your behalf for the cost of hospital-related expenses. If you are not fully covered by your insurance you will be required to pay the estimated ‘out-of-pocket' costs at the time of admission. If you are unsure of your level of cover, or have only taken out health insurance in the last twelve months, please contact your insurance company prior to your hospital stay.

Please ensure that you have correctly completed the health insurance section of your Patient Registration Form.

If you do not have private health cover, you will be required to pay your estimated account prior to or at the time of admission. If your treatment costs exceed the estimated account, you will be asked to pay the difference on discharge. Epworth Freemasons accepts credit cards and has EFTPOS facilities.
